A Case of Chronic Fatigue Treated with Classical Formula

A 38-year-old software engineer came to the clinic with a complaint that had stumped multiple specialists: profound, debilitating fatigue that had persisted for over two years. Blood tests were normal. Thyroid panels were unremarkable. Infectious disease workups came back clean. She had been told, more than once, that the problem was stress, or depression, or simply in her head.

Beyond the Lab Results

From a TCM perspective, her presentation told a clear story. Her pulse was thin and weak (细弱). Her tongue was pale with a thin white coating and teeth marks along the edges. She woke feeling unrefreshed, needed naps in the afternoon, and had a poor appetite despite maintaining her weight through willpower rather than genuine hunger.

The teeth marks on the tongue edges were the key diagnostic sign. In TCM, this pattern indicates spleen Qi deficiency — the digestive system was failing to extract adequate nourishment from food, leaving her body chronically underfueled. The thin pulse confirmed depletion of both Qi and blood.

Choosing the Formula

The classical formula selected was Gui Pi Tang (归脾汤), a prescription from the Song dynasty that specifically addresses the combined pattern of spleen Qi deficiency and heart blood insufficiency. The formula contains herbs like Dang Shen (党参) to tonify spleen Qi, Huang Qi (黄芪) to lift depleted energy, Long Yan Rou (龙眼肉) to nourish heart blood, and Suan Zao Ren (酸枣仁) to calm the spirit and improve sleep quality.

The genius of Gui Pi Tang lies in its architecture. It does not simply stimulate energy. It rebuilds the capacity to generate energy by restoring the spleen’s transformative function, while simultaneously calming the mind and replenishing blood stores. It addresses both the physiological and the psychological dimensions of fatigue.

The Treatment Course

The patient took the decoction twice daily. By the end of the first week, she reported that her afternoon energy crashes were less severe. By the third week, she no longer needed naps. By the sixth week, she described a steady, reliable energy that she had not felt in years.

The tongue changes were equally telling. The teeth marks gradually softened. The pale color gave way to a healthier pink. These objective signs confirmed that the underlying deficiency was being corrected, not merely masked.

What This Case Teaches Us

Chronic fatigue is one of the most common complaints in modern clinics, and one of the most frustrating for both patients and practitioners when standard tests show nothing wrong. TCM offers a framework for understanding why: normal lab results measure specific parameters, but they do not capture the functional capacity of the digestive system to produce Qi and blood.

A car with a full tank of gas but a failing fuel pump will not run properly, even though the fuel gauge reads normal. The spleen is that fuel pump. When it underperforms, the body has access to nutrients but cannot effectively convert them into usable vitality.

The patient in this case was not imagining her fatigue. Her body was genuinely depleted. The classical formula did not give her a temporary boost — it restored her internal capacity to generate the energy she needed.
