Small-Dose Buzhong Yiqi Tang for Intractable Abdominal Bloating

Last month, during an outpatient clinic, a middle-aged man arrived early to queue for registration. His chief complaint was abdominal bloating — present from the moment he woke up, especially pronounced in the upper abdomen. Eating made it unbearably worse, yet skipping meals left him painfully hungry. He was truly miserable.

Through the four diagnostic methods of observation, listening, questioning, and pulse-taking, I found that he had long-standing loose stools, an enlarged tongue body with a thin white coating, a preference for cold beer, dry mouth, and a fine, slightly slippery pulse.

Reviewing his medical records, I saw that over the past two months he had sought treatment at multiple renowned hospitals — Guang’anmen Hospital, Dongfang Hospital, Xuanwu Hospital, and Youyi Hospital — receiving both Western and Chinese medicine interventions, all with unsatisfactory results.

I carefully examined the previous Chinese herbal prescriptions he had taken. They were mostly variations of strengthening the spleen and resolving dampness, soothing the liver and regulating qi, or breaking blood stasis with acrid dispersing herbs. Although spleen-strengthening methods were not underutilized, the results always felt like they were missing the mark.

I was also somewhat at a loss. This condition is genuinely difficult to treat. In the past, using modified Zhizhu Pill would often show initial improvement but poor long-term results. The overall principle still needed to tonify while simultaneously ensuring free flow.

Then a flash of inspiration struck — Buzhong Yiqi Tang came to mind. But considering that he was too deficient to tolerate standard tonification, I decided on a very small dose:

Chaihu (Bupleurum) 2g, Shengma (Cimicifuga) 2g, Zhi Huangqi (Honey-fried Astragalus) 5g, Dangshen (Codonopsis) 3g, Chao Baizhu (Stir-fried Atractylodes) 3g, Danggui (Angelica) 6g, Chenpi (Tangerine Peel) 6g, Fuling (Poria) 6g

I asked him to decoct the herbs himself and drink only 50 ml half an hour before each meal.

After one week, he didn’t return for a follow-up visit. I assumed the treatment had been ineffective and he had simply stopped coming. But the following week, he brought his father in for a different condition and reported that by the third day of taking this small prescription, his digestion had completely normalized. His abdomen no longer bloated at all, and he felt very comfortable.

From this we can see that much of the ancient wisdom is well worth studying. Li Dongyuan’s original doses in Pi Wei Lun (Treatise on the Spleen and Stomach) were very small, and the formula was meant to be used as a powder. Modern clinical doses are at least ten times larger than the originals. While these larger doses do produce results, sometimes smaller doses may actually work better.

Of course, although this treatment was quite successful, similar cases of abdominal bloating remain genuinely difficult to cure. I must continue to work hard!

小剂量补中益气汤治疗顽固性腹胀

上个月门诊时有位中年男士很早就来排队挂号,主诉是腹胀,早上起来就胀,上腹尤甚,吃点东西就胀的不行,可是不吃就饿,十分痛苦。

通过望闻问切,发现此人大便长期不成形,舌质胖大,苔略白,喜欢喝凉啤酒,口干,脉细略滑。

观其病案,近两个月以来已经在广安门医院、东方医院、宣武医院、友谊医院等多个知名医院经过中西医多方治疗,效果不佳。

仔细看了看之前吃过的中医方子,无非是健脾化湿、疏肝理气、破血辛散之类,虽然没少用健脾之法,但总感觉每到点子上。

我也有些不知所措,这种病确实不好治,以往用枳术丸加减往往前期有效,远期疗效欠佳,总的意思还是要补中有通。

此时灵机一动,想到了补中益气汤,又想到虚不受补不可正常补之,故用临时决定用小剂量。

柴胡2g 升麻2g 炙黄芪5g 党参3g 炒白术3g 当归6g 陈皮6g 茯苓6g

要求自己煎药,每天饭前半小时喝50ml即可。

一周后此人未来复诊,当时我还以为可能是没有效果不来了,结果第二周带他父亲来看别的病,说吃了这副小药第三天就完全正常了,肚子再也不会胀了,非常舒服。

由此可知,古人的很多经验都是非常值得学习的,李东垣在脾胃论原方中使用的剂量就非常小,而且是用作散剂。现代用药剂量较原方至少大了10倍,效果是有的,但是有时候可能小剂量更好一些。

当然,虽然这次治疗很成功,但是类似的腹胀依然是很难治。
