On the Similarities and Differences in the Development of Chinese and Western Medicine (14)

Both Chinese and Western medicine are important therapeutic approaches in China. Yet at their core, they emerged as two fundamentally different intellectual systems during their formative and developmental phases. Below are what I consider the key reasons for their divergent paths.

First, geographic differences. Western civilization originated along the coastlines of the Mediterranean and Aegean Seas. The fragmented geography fostered a passion for seafaring among Western peoples, which, over time, quietly cultivated a distinctive spirit of exploration. This spirit profoundly influenced the birth and development of modern Western medicine. In contrast, China’s vast territory and abundant resources gradually gave rise to a conservative temperament. While this trait deeply shaped China’s agrarian civilization, it also placed constraints on the breadth and scope of Chinese medicine.

Second, the profound influence of historical and cultural factors. Historically, the West underwent democratic revolutions relatively early, and its ideals of freedom and democracy promoted the advancement of the natural sciences — including medicine. Ancient China, however, was governed by an autocratic centralized system for millennia. Thought was suppressed under authoritarian rule, distorting the intellectual landscape and objectively hindering innovation in Chinese medicine. Culturally, Chinese medical theory is rooted in ancient Chinese civilization, endowing it with a distinctly humanistic character. Ancient Chinese philosophical concepts — such as yin-yang and the five elements — form the philosophical foundation of Chinese medicine, which is represented by xiang (image-based) thinking. Western medical thinking, by contrast, traces its origins to ancient atomism, focusing on the properties of individual substances. Combined with mechanistic philosophy, this gave rise to a theoretical foundation grounded in reductionism.

Third, differences in clinical reasoning. Guided by xiang thinking, Chinese medicine emphasizes a holistic approach, employing pattern differentiation and treatment (bian zheng lun zhi) through the four diagnostic methods. Throughout the entire therapeutic process, abstract thinking prevails — making Chinese medicine essentially a dialectical medicine. Western medicine, guided by reductionism, starts from individual organs or localized areas, directly targeting the site of disease through examination and laboratory results. In essence, it is a disease-differentiation medicine.

Fourth, the catalytic role of catastrophe. After the devastation of the Black Death in the fourteenth century, the West came to recognize the futility of religious and theological approaches to healing. This served as an objective catalyst, pushing Western medicine toward greater emphasis on empirical observation and objectivity. China, however, experienced no such catastrophe, and this absence may bear a certain necessary connection to Chinese medicine’s failure to embrace a more direct, observational approach.

中医与西医两者在我国都是重要的医疗手段,但归其根本,两者在早期形成与中期发展的过程中都是两种绝然不同的学术体系。以下便是我以为其发展迥异的原因。

其一,两者地理环境的不同,西方文明的发源地为地中海与爱琴海沿海,其支离破碎的地理环境使西方人热衷于航海,久而行之便潜移默化的培育了其特有的探索精神,这种精神对后来西方现代医学的产生与发展有很大影响。反观中国,地大物博,领土广袤的优势却使国人不知不觉形成了保守性这一特质,这一特质虽对中华农业文明有深刻影响,但是对中医的视野起到了限制作用。

其二,受历史因素与文化因素影响颇深。在历史因素上,西方较早的进行了民主革命,其自由民主的思想对其自然科学的发展有促进作用,当然对西方医学的发展有促进作用。而中国古代一直为专制主义中央集权制度,思想长期受到铁血压迫,使国人思想扭曲,客观上不利于中医的创新发展。从文化上来说中医理论起源于中国古代文化,从而使中医具有极其鲜明的人文医学特征。中国古代哲学思想,如阴阳,五行等构成了以象思维为代表的中医理论的哲学基础。而西医思维起源于古代原子论,侧重与某一物质的特质,加上机械的思想便形成以还原论为特质的西医理论基础。

其三,中西医的临床思维不同,中医是在象思维的指导下强调从整体出发,运用辨证论治的方法,通过四诊查证,在治病全程都是运用抽象思维,属于辩证医学。而西医是在还原论的思想指导下,从某人器官或称作从局部出发,直击病变部位,通过检查化验结果,本质上属于辨病医学。

其四,西医在十四世纪经历了黑死病的肆虐之后认识到了宗教神学装医治病的愚昧性,从而作为一个客观催化剂促进了西方医学更加注重客观性的发展。而在中国却没有这一大灾难,这与中医始终未走上直观性的医学上有某种必然的关系。
